If you are living in a care or rehabilitation facility, and want to move into the community, the discharge process can be very challenging. BCITS can work with you to facilitate your transition to living in the community.
If you are a ventilator user, most care facilities, including GF Strong Rehabilitation Centre, know about our Provinicial Respiratory Outreach Program (PROP) and most likely will ask us to become involved in discharge planning. If you are currently a resident of the George Pearson Centre, a peer from the Community and Residents Mentors Association (CARMA) can support and advise you during your discharge.
